I hope this helps you

y=-1/4x+1 line slope is m=1/4

perpendicular lines slopes multiples -1

m.m'=-1

1/4.m'=-1

m'=-4

(5,21) x'=5 y'=21

m'. (x'-x)=y'-y

-4. (5-x)=21-y

-20+5x=21-y

-y=5x-41

y=41-5x
