Since it is each month and membership fee is ONE time, the equation is 14x+25=y. 
Since it is asking for 6 months worth of cost, we will input the 6 into the x. 
14(6)+25=$109.
It's pretty expensive, if you ask me.

25 + 14m= the cost
25 + 14(6) = 109
